Please read these instructions (as of November 12, 2010) before downloading the two page Registration Form.
1) The Pilgrim is to complete the top part of page one. The Sponsor is to complete page two. The Sponsor will mail in both pages to the Registrar (address at bottom of page two) along with a $10.00 deposit check.
2) Please answer ALL the questions. Also, please PRINT LEGIBLY.
3) The information you share is for placement only and will be held in strict confidence. Things like your name, address, telephone number, Email address, etc., will be added to the N.W.O. Emmaus Community Data Base. This Data Base is available to all Community members on this web site and can be used by you and others to call each other, write notes of encouragement, etc., and will be used by the Outreach person on the Emmaus Board to communicate prayer requests and reminders about various Community events via Email to all Community members.
4) All other personal information (medications, diet restrictions, health problems, physical handicap, etc.,) will only be used to help the Team Leaders of the Walk to make the Pilgrim's weekend as enjoyable and as safe as possible. When the Walk is completed, the Registration Forms and other documents prepared from the Forms will be shredded and thrown away.
5) To partially offset the costs of the Walk, each Pilgrim will contribute $70.00 (checks only please). In addition to the deposit mentioned above in item #1, the remaining $60.00 is due when the Pilgrim arrives on the Thursday evening of the Walk.
6) All Registration Forms should be mailed to the Registrar no later than 14 days before the Walk begins. This is necessary to allow the Team Leaders to plan appropriately for the weekend. Pilgrims will be invited to the next Walk (about 5 weeks before it begins) in the chronological sequence in which Registrations are received (with a few exceptions). However, God’s grace (invitation to Walk) will be shown to any pilgrim who at the last minute decides to attend if space is available (although this is NOT encouraged).
7) Left click once on the line below entitled “Download” to download the Registration Form. Left click your “Print” icon at the top of the page and the two pages will printout automatically (you should not have to set print ranges, fonts, etc.).
